上顎洞底挙上術の基本
上顎洞底挙上術は局所麻酔下で行うことができます。 歯槽骨側の残存骨高が4-6 mmの場合、外部洞底挙上術が適応とされ、この場合は骨造成とインプラント埋入術の同時施行が可能です。 残存骨高が4 mm未満の場合、上顎洞底挙上術の後にインプラント手術を行います。 初期の固定が得られる場合、インプラント手術を施行することができます。
Sinus floor elevation
An external sinus lift can be performed under local anaesthesia. If the dorsal residual bone height is 4-6 mm, an external sinus lift is indicated, sometimes with primary implantation. If the dorsal residual bone height is <4 mm, an external sinus lift with secondary implantation is indicated. Primary implantation can be performed when primary stability has been achieved.
インプラント治療の自験例
自験例です。
抜歯・骨造成・インプラント埋入の同時施行術です。8年経過し、現在も安定しています。骨を2～4㎜ほど造成したことでインプラントの安定性を高めています。
また、インプラント治療すべてに言及できますが、お口全体のかみ合わせについても必ず診査しなくていけません。前歯部の治療も同時並行で行っています。
自験例（続き）
CTによる画像確認と術前のシュミュレーションを行います。シュミュレーションの位置をより正確に再現するため、サージカルガイドを製作する場合もあります。インプラント治療は時間がかかります。治療開始の時期が大切になります。インプラント治療を故意に遅らせることで、かえって治療のリスクが増加することもあります。
